When the penis is very crooked, the man may suffer from a problem of congenital penile curvature, that is, a birth defect that starts to be noticed, however, only in puberty and adolescence, when sexual life begins. However, the main cause, says Júlio, is Peyronie's disease. “The most accepted theory is that microtraumas to the penis (small bruises, blows) induce marked inflammation, which generates abnormal scarring in the tunica albuginea (cover of the corpora cavernosa responsible for erection).
Hence, these scars result in calcified plaques, which alter the elasticity of the tissues, causing, among other problems, accentuated penile curvature during erections”, he explains.
Rodolfo Favaretto Filho, specialist doctor at the Brazilian Society of Urology, adds: “Microtraumas can appear due to intense sexual activities, application of intracavernous medications to stimulate erection, fractures of the penis (common to occur when there is poor quality of erection of the organ, which not so rigid, you can suffer injuries when penetrating), among others”.
If a man is diagnosed with Peyronie's disease, when he feels his erect penis, he will feel the hardness of the calcification. With it flaccid, you will notice a firmer consistency to the touch. In congenital penile curvature, there is no sign of hardened plaques that are palpable or visible in diagnostics. When the problem is congenital, the penis is usually large and curved downwards, but it can also be turned to all sides.
Position inside underwear does not affect shape
As for the positioning of the penis inside the underwear, don't worry, because even if it is a little crooked when flaccid, its flexibility guarantees adjustments in any direction. Keeping it on the opposite side to its curvature will also not result in any change in shape, as, with walking and the natural movement of the body throughout the day, the organ can change position by itself.
When urinating, depending on the angle, it can be a little difficult to get the toilet right, but the deformity rarely compromises the functioning of the urinary system.
And don't think that this is a subject that only concerns big boys. Urologist Rodolfo Favaretto Filho points out that in the case of penile curvature related to Peyronie's disease, when there is an accumulation of microtraumas, older men flee. “It affects approximately 10% of men and, more frequently, those over 40 years of age. But the curvature can appear in young people, too, especially after the start of sexual activity”, he explains.
What is crooked can be straightened
To resolve the issue of tortuosity, if there is pain, discomfort and the doctor determines the need for intervention, the patient can undergo treatments or surgeries.
“In the acute phase, when there is inflammation, it is possible to stabilize the fibrotic plaque and, as a consequence, penile tortuosity, with the use of oral medications, gels, ointments and even injectable drugs”, explains Rodolfo, about treatments for the disease. of Peyronie. It is not possible to treat congenital curvature with medication, as, in this case, there was no trauma or previous infections.
Still on treatments for Peyronie's disease, urologist Júlio adds: “With shock waves from devices, originally used to break up kidney stones, it is also possible to reduce the size of the plaque and its associated deformities, including erectile dysfunction.”
Surgical procedures, in turn, come into play only when the curvature is congenital, or when (in case of Peyronie's disease) the plaque is already calcified and the penile curvature has been stabilized for at least six months. Depending on the case, penis size reconstruction can be performed to ensure patient satisfaction and even a penile prosthesis can be placed when there is associated severe erectile dysfunction.
If the reason is Peyronie's disease, in order to avoid having to undergo surgery, doctor Rodolfo Favaretto Filho concludes: “All risk factors that could generate penile microtraumas should be avoided, which involves vigorous sexual activities or with sexual activity. partially hard penis, in addition to correcting causes of erectile dysfunction, such as diabetes, cholesterol, hypertension and excessive consumption of alcohol, cigarettes and fatty foods”.
