Psychologist Pedrosa Answers: I can only reach orgasm when I masturbate, what should I do?

Hello Dr. Pedrosa, good evening! My name is Nilton, I'm 21 years old, I'm homosexual and I've read a lot about the situation I find myself in, including on your website. What happens to me seems to be a case of Delayed Ejaculation, I definitely cannot ejaculate during sexual intercourse with my partner, only when I masturbate, I have no inhibitions, and I masturbate next to him right after he reaches orgasm.

I don't follow any religion, my family accepts my sexuality, I don't have diabetes or any disease that could characterize an "organic" factor, on the other hand, I masturbate a lot, about two or even three times a day, when I masturbate I can reach the I orgasm easily, during penetration I don't feel enough pleasure, I read that masturbation may have taken away the sensitivity of my penis. What to do? Can stopping masturbation guarantee an improvement in my condition? Any medicine? Thank you in advance. Nilton (São Paulo – SP)

According to your report, you seem to have Delayed Ejaculation. This sexual dysfunction is not very common, such as Rapid Ejaculation and Erectile Dysfunction, which are more prevalent in heterosexuals and homosexuals. All sexual dysfunction can have an etiology linked to organic factors (when a clinical condition linked to a disease interferes with sexual performance) or behavioral factors (when certain individual behaviors and their sexual environment interfere with their performance). An example of an organic factor is a diabetic subject who will have erection problems. The cause of sexual dysfunction is linked to disease.

Regarding the behavioral factor, also called non-organic, it seems to be your case. At 21 years of age, you probably don't have any chronic illness, but you've made a habit of masturbating three times a day. You have created a sexual environment where you do not feel pleasure from penetration. You can only orgasm and ejaculate with your hand during masturbation. He sensitized his penis with his hand, that is, he "got used to" achieving orgasm with his hand.

The sensitivity of the penis remains the same. No medicine needed. The way for you to resolve your difficulty is to stop masturbating for a minimum period of six months. Going into extreme deprivation, you will begin to ejaculate upon penetration. The first two times you will have some difficulty, then you will succeed. If you cannot ejaculate during penetration, do not masturbate. Try several times until you are able to ejaculate upon penetration. Important: once you start ejaculating during penetration, do not masturbate again, as masturbation has already become extinct. If you start masturbating again, the difficulty will return with full force. Good luck!
